Please complete the registration form below––one registration form submission per person. Indicate the test(s) you would like to purchase by checking the box in front of your selected test(s). The system will tabulate your total and provide you with a recap prior to your final purchase. Visa, MasterCard, Discover and American Express are accepted.

Your receipt will be sent to the e-mail address you provide on the registration form. Tests are provided on a walk-in basis during normal laboratory business hours. Please print the receipt when you're ready and bring it with you to one of our two outpatient labs––one in our Noblesville hospital and one in our Westfield hospital.

Outpatient Laboratory Locations

  • Noblesville Hospital––Park and enter through Entrance #1 and check in at registration around the corner from the ER lobby.
  • Westfield Hospital––Park and enter through the Main Entrance and check in at registration.

Lab hours for both locations:

Monday-Friday: 7 a.m. to 6 p.m.
Saturday: 7 a.m. to 12 p.m.

Baseline EKGs
Baseline EKG appointments are only for adults age 18 and older and are only performed Monday-Friday from 8 a.m. to 4 p.m. After you make your purchase, print your receipt and call 317.776.7444 to schedule your EKG. When it's time for your appointment, take your receipt and report to the Courtney Cox Cole Infusion Center on the second floor (main floor) of our Noblesville hospital (Entrance 1).

This test is designed specifically for student-athletes. Fasting is not required. This package includes:

  • Complete Blood Count—An indicator of general health, this test counts the white cells, red cells and platelets in blood.
  • Iron Panel—This measures the level of iron in the bloodstream.
  • Ferritin—The results of this test reflect the amount of stored iron in the body.
  • Sickle Cell Screen

This screening is for adults age 18 and older. With a baseline EKG screening, the electrical activity of your heart is recorded on paper and read by your healthcare team. The pattern of your heart signals can tell your doctor whether your heart is normal, under stress, or experiencing strain or damage. At Riverview Health, a computerized system stores your baseline EKG keeping it readily available for your healthcare team.

This test is performed to test kidney function

This test determines your blood group and Rh type. (test: ABO/RH)

The celiac disease test is used to determine someone’s sensitivity to gluten, a protein found in grains like wheat, barley or rye. When someone with celiac disease eats gluten, the immune system reacts which may lead to intestinal discomfort and may eventually damage the intestinal wall.

This test is designed specifically for children. Fasting is not required. Please note this test requires a regular blood draw. This test includes:

  • Basic Metabolic Panel
  • Complete Blood Count with White Blood Cell Differential and Platelets
  • Cholesterol/Lipid
  • Hemoglobin A1C

Complete Blood Count with differential and platelets

This is used as a broad screening tool to evaluate or monitor organ function and check for conditions such as diabetes, liver disease and kidney disease.

This test is a nasal PCR test.

This panel includes:

This test measures antibody levels to determine if your immune system can respond against an infection caused by measles.

This test measures antibody levels to determine if your immune system can respond against an infection caused by one of these diseases.

This test measures antibody levels to determine if your immune system can respond against an infection caused by mumps.

Please fast for 12 hours for the most accurate results. This profile is designed to evaluate your current nutrition status and includes:

  • Cholesterol/Lipid
  • Complete Blood Count with Platelet
  • Folate
  • Vitamin B12

This test may be helpful for patients wanting to monitor hormone health and changes as they move through menopause. All results in this profile should be interpreted with the help of your healthcare provider.

  • Estradiol
  • Follicle Stimulating Hormone (FSH)
  • Luteinizing Hormone (LH)
  • TSH (If abnormal, FT4/Thyroxine is performed)

This test may be helpful for patients wanting to know more about their hormone health and changes during the pre-menopausal stage. All results in this profile should be interpreted with the help of your healthcare provider.

This tests for Prostatic Specific Antigen

Allergies are the body’s response to increased exposure and reaction to common substances in the environment or in food called allergens. Windblown pollen from grasses, trees weeds, animal hair, house dust, foods and many other substances are examples of allergens.

Blood allergen testing determines an individual’s sensitivity to a given allergen. The test measures immunogloblulin E, and antibody in the blood when the body is fighting an allergen.

This panel includes:

  • Total IgE
  • 6 common Grasses/Weeds, including Ragweed
  • 10 common trees
  • Molds
  • Pet Dander for cats and dogs
  • Dust mites

This test measures antibody levels to determine if your immune system can respond against an infection caused by rubella.

This test evaluates thyroid function and/or symptoms of hyperthyroidism or hypothyroidism. (tests: TSH and free T4)

This test measures antibody levels to determine if your immune system can respond against an infection caused by varicella (chickenpox).

This test is used to determine if you have a Vitamin D deficiency or if you are taking a Vitamin D supplement, to determine if your Vitamin D supplementation level is appropriate.

This profile shows important information about your kidney function, blood sugar, and electrolyte levels and acid/balance base, as well as your risk of developing coronary heart disease.

Test Key

Anemia (Iron Panel and HGB)—Anemia is a condition that occurs when the number of red blood cells and/or the amount of hemoglobin found in the red blood cells drops below normal. This test measures hemoglobin and evaluates the iron stores in the body.

Complete Blood Count—As an indicator of general health, this test counts all the cells that make up your blood. White cells, red cells, and platelets are all evaluated in this test.

Diabetic A1c—This test evaluates blood sugar control for individuals who have, or are at risk for, diabetes. The A1c test shows the average glucose in the blood during the last two to three months. The Estimated Average Glucose (EAG) is provided with the A1c test and screens for diabetes and pre-diabetes.

Baseline Electrocardiogram (EKG) Screening—With a baseline EKG screening, the electrical activity of your heart is recorded on paper and read by your healthcare team. The pattern of your heart signals can tell your doctor whether your heart is normal, under stress, or experiencing strain or damage. At Riverview Health, a computerized system stores your baseline EKG keeping it readily available for your healthcare team.

Glucose—This test determines if your blood glucose level is within healthy ranges and tests for hyperglycemia, hypoglycemia, diabetes and pre-diabetes.

Heart health (Lipids, hs-CRP and Homosysteine)—This group of tests is used to assess the risk of coronary heart disease. The lipid test includes total cholesterol, HDL (high density lipoprotein cholesterol), often called good cholesterol, LDL (low density lipoprotein cholesterol), often called bad cholesterol, and triglycerides.

Hemoglobin (HGB)—This test measures your amount of hemoglobin, a part of the red blood cell that carries oxygen from the lungs throughout the body. It also returns carbon dioxide back to the lungs to be exhaled from the body. If your hemoglobin levels are low, you may have anemia, a condition in which your body is not getting enough oxygen, causing fatigue and weakness.

High-sensitivity CRP—The high-sensitivity CRP level is used to assess the risk of developing cardiovascular disease.

Homocysteine—This test measures your levels of homocysteine, an amino acid that is produced by the body, usually as a byproduct of consuming meat. Elevated levels of homocysteine in the blood may be associated with atherosclerosis (hardening and narrowing of the arteries) as well as an increased risk of heart attack, stroke, blood clot formation and possibly Alzheimer’s disease.

Kidney function (BMP)—This test measures current status of your kidneys, blood sugar, electrolyte, and acid/base balance. This test also measures albumin and phosphorus, which test for liver or kidney disease.

Liver function—This test screens for liver inflammation and damage.

Prostate (PSA)— This test evaluates prostate health and can identify individuals at risk for prostate cancer.

Thyroid (TSH and Free T4)—This test evaluates thyroid function and/or symptoms of hyperthyroidism or hypothyroidism. The test includes a thyroid stimulating hormone test and a free T4 test. T4, or thyroxine, is a hormone produced by the thyroid gland.

Vitamin B12/Folate—B12 and folate are B complex vitamins that are necessary for normal red blood cell formation, tissue and cellular repair, and DNA synthesis. This tests for deficiencies in either B12 or folate.
