Anticoagulation monitoring is the primary service provided by Riverview Health Medication Management Clinic. People with a wide range of health conditions are prescribed warfarin (Coumadin, Jantoven) to help prevent excess clotting. Those who take warfarin require regular monitoring to ensure they have the appropriate degree of anticoagulation, or “blood thinning,” without experiencing unwanted side effects. Many factors can affect how much warfarin a person needs, and routine blood testing is required to ensure safe and effective use of this critically important medication.
When you visit Riverview Health Medication Management Clinic, your first appointment for anticoagulation monitoring will take about 30 minutes. After your initial visit, most of your appointments will only take about 15 minutes.
During your appointment, we use a simple finger-stick blood sample to determine how well your medication is preventing blood clots. A pharmacist will discuss any necessary medication adjustments and provide you with easy-to-understand instructions on how to use your medication safely and effectively.
