Sports Physicals for Westfield Schools

Through a partnership with Westfield Washington Schools, Riverview Health is offering sports physicals for student athletes (grades 6-12) at a cost of $30 per person.

Each 15-minute time period is limited to 20 students, and registration is required. Walk-ins will not be accepted due to COVID-19 guidelines.

In order for a student-athlete to be eligible for any sport, a completed sports physical form must be returned to the appropriate athletic office prior to the first practice. The examination must take place—and the provider’s signature must be dated—after April 1, 2025. Sports physical forms are available through your school's athletic department.

Please complete the form with the student’s information. You’ll receive a receipt via email, sent to the email address you provide. Please print and bring the receipt with you to the sports physical.

Please note these will be the only student-athlete physical examinations offered through Westfield Washington Schools for the 2025-2026 school year.

Registration will close at noon on Tuesday, May 20th.
